The user must authenticate twice to access the application SSO/APM

We are facing a problem related to the SSO on APM for application. The user must authenticate twice to access the application.   someone help me? Thank you.

Guys, thanks for the help found the problem. He was in the "Forms" uri the address was wrong. Sorry for the trouble exemplify what happened. But it was solved. Thank you.
