Report F5 devices,Need script

we have to fetch the report from with below info :

 

VIP Name Service port of VIP Pool member deatils with IP Pool memeber service port Pool member status

 

Pls let us know if you have.

    the traffic map (first option under local traffic) seems like a pretty close match for this, or do you need it in some other format?
