It really depends on what features you want or what you might be working on in production if you have any. So far, I really like 12.0 HF3. This seems to be the most stable version for me.
depends on your hypervisor at first, so choose vmware, xen, ...
then probably the ALL image, but if you are sure you only use LTM you could do that one.
you didn't ask i believe but versionwise 12.1 latest hotfix or if you want to be very safe 11.5.4 with latest hotfix.

In my opinion:
1) NEVER run a ".0" release of ANY software in production.
2) ALWAYS wait for a Long Term Support (LTS) release.
There are companies / consulting firms where breaking either of the above is an automatic disciplinary investigation.
If you are installing in a lab / sandbox environment, then chase and investigate features, but in production, most enterprises value stability above all else...
